Appendix 2: Congregational Survey

We are in the process of forming a recovery ministry to provide support and encouragement to interested members. If you are interested in participating in the programs that will be offered, please help us by completing this survey.

The survey is anonymous, and will be used solely to determine the specific needs of the congregation members who wish to participate.


It is our sincere desire to create an environment where people can attend Christ-centered support group meetings as part of their commitment to heal their hurts.

Please check the behaviors that currently apply to you.

    Excessive use of drugs ( ) alcohol ( ) food ( ).

    Over-indulgence in sex ( ) work ( ) gambling ( ) spending ( ).

    Compulsive behavior expressed through excessive volunteering ( ) caretaking of others ( ) perfectionism ( ) self-improvement ( ).

    Obsessive focus on sin ( ) weight ( ) pornography ( ) status ( ) relationships ( ).

    Unreasonable fear of crowds ( ) rejection ( ) sex ( ) public speaking ( ) failure ( ).

    Excessive attention and focus placed on others as a means of establishing identity and self-worth ( ).

If you are in a relationship with someone who is not completing this survey, please check the behaviors that you feel apply to him or her.
